Medical Clearance for Freshmen/Transfer Student Athletes
Please contact your head coach and ensure that you are on their current Frontrush Roster. They need your name, DOB, cell phone #, and Bridgewater State University email address. Please follow the instructions in the email from j5obrien@bridgew.edu subject: BSU Medical Clearance 20-21 ... to create your frontrush account and fill out the required forms (compliance form and sports information form).
Log into your Bridgewater State University Email. You will receive an email from heaoperation@cerner.com. Check your junk mail if you do not see it.
Please follow the instructions in the heaoperations email.

There are five 2020-2021 Forms that need to be filled out completely.
Once you finish a form, press Home; you will be taken back to the page with the rest of the forms needing completion. To see which forms you have completed, click the Forms tab. If a form is in “Saved Forms” it has not been completed.
On the 2020-2021 Student Athlete Health Questionnaire, you must fill in N/A in fields that are not applicable to you in order to completely submit.
Once you finish your forms, please upload your most recent physical in the Documents tab.
You can either scan or take a clear picture of your physical with your smartphone (make sure you upload every page of your physical). When uploading please name the document "PE-last name-date of physical". The physical must clearly state the physical exam date and that you are “Cleared to participate in sports without restrictions”. Per NCAA guidelines, the physical must be within 6 months of the start of your traditional or non-traditional season. If your most recent physical was performed longer than 6 months ago you will need to provide a new physical. We understand insurance will not pay for an additional physical in the same calendar year, but it is an NCAA requirement that is not negotiable. Call your primary care physician to see how much an out of pocket physical will cost compared to a medical clinic physical (For example, CVS Minute Clinic physicals are approximately $75.00 and some healthcare organizations offer sports physicals for a lower price in August). Please upload a current copy of the front and back of your insurance card in the Documents tab. Please name the image of the front “InsCardFront20-21” and the image of back “InsCardBack20-21”. **Helpful Hint: Iphone users, go to Notes, click on the camera, and use the scanner feature for perfect uploads
Please upload any new doctors notes from recent injuries, concussions, surgeries, clearance notes for return to play etc. If you need a Referall form they can be found here.
